This is a piano solo performance event by students at seven different levels, to be judged individually on a number basis from 1+ to 4. Students perform individually. Forum Auditions is an event that requires a high standard of preparation and performance from the student in order to achieve a superior rating. Judging standards are consistently high in order to challenge the student and teacher to work towards their musical potential.

- Fill out the entry form
- At the bottom of the form, click the green “Download” tab to save the form onto your computer. Then, click the blue “Email to Admin” tab.
- Technology is unreliable, so please also email the entry form to sarahHkuo@gmail.com.as an attachment by Nov. 3, 2021.
3. Mail one check (made out to FORUM) for all your students ($30 x total number of student entries) to the following address: Phebe Wusan 19 Town Oaks Place Bellaire, TX 77401. You may also give the chairperson your check on or before the deadline.
4. Turn in all completed materials including the Authorization for Photo Release Form above to the chairperson by November 3, 2021. If mailing or emailing registration, please do so ahead of time to ensure the chairperson receives it by the deadline.
5. Please note: You will be required to fill out repertoire and scale information for each student registered. Please refer to the Performance and Technical requirements.
Teachers are responsible for filling out their students’ critique sheets. Critique sheets can be downloaded from the Forum Auditions tab on the Teacher Dashboard.

Students must play the technical requirement of their respective level or a higher level. Grading of the scales will be judged as being satisfactory or unsatisfactory. A grade of unsatisfactory will result in lowering the rating by one increment. (For example a 1+ will be lowered to a 1, 1 to a 1-)
